Diabetes Management

What is a Diabetes Management Plan?

This plan outlines the type of diabetes the client has and the management of the condition.  It will include any medications the client is taking for the management of their diabetes, when the client should be taking their medication and when they need to be recording their blood sugar levels.  This plan may also consist of a diet the client will be expected to follow. This plan will also identify what to do in the event if a client’s sugar levels are too high or too low.
