NDIS Goal Setting sheets

Setting your NDIS plan goals

We can develop goals sheets to be used for the preparations for a client’s NDIS Plan or to just identify some goals and a plan of action to be implemented to assist the clients to achieve these goals.

Part of this process would be a meeting with the client and their family (and sometimes their day program or school as well) to identify some goals for the client and to agree on a plan to implement those goals. Including the school or day program can be useful as often a client may behave or enjoy activities at school or day placement they do not want to do at home. Or it maybe they enjoy doing a activity with a certain staff member or family member but are not interested in doing the activity with a different person.

The goals can be divided into short, medium- and long-term goals and can fall into several different categories. The data collection sheets for the goals will also be created so the goal can be measured to assess whether it is working or not working for the client and whether the plan needs modification, or the goal needs to be reassessed.
