Communication Plan

What are the benefits of having a communication plan?

This document identifies a client’s communication style.  This plan will outline the client’s level of understanding of expressive and receptive language.  The plan will also outline any communication aids that a client maybe using.  Such as a communication board, flash cards etc.

Being able to communicate helps a client to feel less isolated and is an important aspect in forming relationships with others.  Being able to effectively communicate assists a person in being able to get their needs met. Communication takes many forms and much of our communication is non verbal cues such as body language.

This document is often written by a speech therapist but can be written by someone who knows the client well.  This document will identify how a client likes to communicate, and outlines strategies on how to best communicate with the client.  E.g. with gestures, key words, direct eye contact etc.

A client under NDIS may require assistance in the areas of communication and having a good communication plan and data collection on the interventions that have already been implemented also supports a client’s claim for funding.  It is also important to outline the impact on the client’s life of not having the intervention in place to further support the claim.

Resource for Medical Emergencies

In the event of a medical emergency a client’s care plans and case management form an excellent resource for medical staff to treat a client. The care plans outline what care a client is currently receiving and a client’s medical history.

Plans document what treatments have been implemented for a client both their current interventions as well as past interventions. This assists medical and other specialists in determining a treatment plan for the client.  It informs them on what worked for the client previously and what has not worked.

Enables Specialist Collaboration0

Enables Specialist Collaboration

By having the client’s care requirements documented it also provides the information to medical and other specialists on the other treatments a client may be receiving from another specialist. This means that all specialists are working collaboratively to provide the best quality of care for the client.
